The Post-Approval Process

Once the program has been approved, the accountable physician will receive an approval letter by mail whose address is provided on the Essential Information Form. CPD will forward your approved accreditation to the appropriate medical college(s).

If you require UBC CPD to print certificates for you, please create and submit an attendance record in excel format for all participants. Click here for sample attendance record. Certificates can be produced prior to or following your program. For more information on certificates, please visit our certificate information page.

For each day that your program runs, you are required to maintain a sign-in sheet for participants. You must also distribute evaluation forms for all participants, and collect them once completed. Click here for a sample evaluation form. For Mainpro-M1 and AMA applicants, the designated CPD representative (as indicated on your application ) is required to complete an audit of your program.


Specific Requirements for Your Program (by credit type)



Credit Type: Royal College of Physicians and Surgeons Maintenance of Certification Section 1

Within three months of program completion, collated responses to the industry bias question on the evaluation forms must be sumbitted to following address or joanna.c@ubc.ca:

Credit Type: College of Family Physicians of Canada Mainpro-M1

  1. Within one month of program completion, the completed audit must be submitted to the following address or joanna.c@ubc.ca:

    2.  Within three months of program completion, collated responses to the industry bias 
         question on the evaluation form must be submitted to the following address or

Credit Type: American Medical Association AMA PRA Category 1

  1. Within one month of program completion, the completed audit must be submitted to the following address or joanna.c@ubc.ca:

  2. Within three months of program completion, the following must be submitted to following address or joanna.c@ubc.ca:

    - a complete evaluation summary, including collated responses to the industry bias question


  3. Submit a request for AMA certificates complete with payment and an AMA attendance record in excel format.
