CFPC Mainpro Credit Submission Forms

There are three ways to enter your Mainpro credits: Online: Go to www.cfpc.ca and click on “Members Area”; follow the directions provided. Email or Fax: Complete this form and scan/email to mainprocredits@cfpc.ca or fax to 1-888-843-2372 (Canada only) or 905-629-0893

Linking Learning to Practice


Earn Mainpro-C credits when you reflect upon issues and questions that arise as part of your clinical or non-clinical activities. For example, the issue or question may be a result of your participation in one of the following activities: Download  Submit online
  Office practice, hospital work, research    
  Teaching, presenting, mentoring    
  Examinations, committee work, accreditation survey    
  Accredited or unaccredited CME/CPD event, annual scientific assembly, hospital rounds    
  Receive 2 Mainpro-C credits (plus 2 bonus Mainpro-M1 credits) for each form you complete and submit.    
  You may submit multiple forms for a single activity as long as each is based upon a distinctly different issue/question.

Earn Mainpro-C credits when you reflect upon practice-based questions that arise in your daily practice. This five-step form captures the question, research used to answer the question, and the clinical decision made as a result of analyzing available information; it also prompts you to assess your decision after an elapsed period of time. Receive 3 Mainpro-C credits (plus 3 bonus Mainpro-M1 credits) for each form you complete and submit.  Download  Submit online
Provincial Practice Review/Enhancement Program



Earn Mainpro-C credits for participating in a provincial practice review or practice enhancement program. Receive 3 Mainpro-C credits (plus 3 bonus Mainpro-M1 credits) for each form you complete and submit. You may submit multiple forms as long as each is based upon a distinctly different question. Practice Audits/Quality Assurance Programs



Earn Mainpro-C credits for participating in a practice audit or quality assurance program. Receive 3 Mainpro-C credits (plus 3 bonus Mainpro-M1 credits) for each form you complete and submit. You may submit multiple forms as long as each is based upon a distinctly different question. Download
Clinical Traineeships & Fellowships





Earn Mainpro-C credits for completing a clinical traineeship or fellowship. To be eligible, the activity must be a minimum of 5 days in duration. Typically, clinical traineeships are 2 to 4 weeks in duration and may occur over an extended time period (e.g. multiple weekends): Receive 25 Mainpro-C credits (plus 25 bonus Mainpro-M1 credits) when you complete and submit this form. Family/Emergency Medicine Examinations Earn Mainpro-C credits for completing a family medicine or emergency medicine examination. The following are eligible examinations:  Download 

CFPC Examination of Special Competence in Emergency Medicine


ABFM Family Medicine Certification and Recertification Examinations


ABEM Emergency Medicine Certification and Recertification Examinations


The CFPC will consider other examinations on an individual basis (e.g., certification examination in forensic medicine). The CFPC Certification Examination in Family Medicine is not eligible for Mainpro-C credits.


Receive 25 Mainpro-C credits (plus 25 bonus Mainpro-M1 credits) when you complete and submit this form.


University Degree/Diploma studies
1. Report and earn credit as you complete individual courses toward your degree/diploma: Download  Submit online
*NEW!*You may now choose to report degree/diploma participation in one of two ways!
  • Each course is worth five Mainpro-C credits AND 15 Mainpro-M1 credits (this includes five bonus Mainpro-M1 credits).
  • Submit one form per course.
   
 (NOTE: Select only one of the options for reporting. You may not use both or a combination of the two) 2. Report and earn credit upon program completion:
   

  • Earn 25 Mainpro-C credits (plus 25 bonus Mainpro-M1 credits) upon program completion.
  • Submit one form per program.
   
CPCSSN Quality Assurance Program











This form documents how you used your experience with the CPCSSN chronic disease profile of your practice to critically reflect on your management of one of the chronic diseases that are under surveillance in your practice by CPCSSN. The QA activity may or may not have included a comparison with other local or national practices. You will receive 3 Mainpro-C credits for each form you submit. You may submit a form once a year for each of the chronic disease under surveillance in your practice by CPCSSN (providing a practice profile of the chronic disease has been created for you by the CPCSSN Database Manager).  Download  Submit Online
Request for Individual Consideration Use this form to request that an unaccredited or non-Mainpro accredited activity be considered for Mainpro credit. Download Submit online
