Clinical
Traineeships & Fellowships
|
Earn Mainpro-C credits for
completing a clinical traineeship or fellowship. To be eligible, the activity
must be a minimum of 5 days in duration. Typically, clinical traineeships are
2 to 4 weeks in duration and may occur over an extended time period (e.g.
multiple weekends): Receive 25 Mainpro-C credits (plus 25 bonus Mainpro-M1
credits) when you complete and submit this form. |

completing a family medicine or emergency medicine examination. The following

CPCSSN
Quality Assurance Program
|
This form documents how you used
your experience with the CPCSSN chronic disease profile of your practice to
critically reflect on your management of one of the chronic diseases that are
under surveillance in your practice by CPCSSN. The QA activity may or may not
have included a comparison with other local or national practices. You will
receive 3 Mainpro-C credits for each form you submit. You may submit a form
once a year for each of the chronic disease under surveillance in your
practice by CPCSSN (providing a practice profile of the chronic disease has
been created for you by the CPCSSN Database Manager). |
