The Division of CPD of the University of British Columbia, Faculty of Medicine (UBC CPD) respects and values your privacy and the confidentiality of the information you choose to share with us. We are committed to protection of this information and will assure your privacy when participating in CPD activities and when accessing information through our Web site.
The UBC CPD provides this Online Privacy Statement to make you aware of our privacy policy and practices and of the choices you can make about the way your information is collected and used. This policy has been created to communicate with you the commitment we have to the safeguarding of your personal information. To make this notice easy to find, we make it available on our homepage and from the bottom of every CPD web page. All personal information is collected pursuant to section 26 of the Freedom of Information and Protection of Privacy Act, RSBC 1996 c. 165.
Important Exclusion: Please note that this policy governs all activities of UBC CPD that are not formal research projects subject to ethical review by The University of British Columbia Office of Research Services. All UBC CPD research activities are governed by the policies and procedures of the University of British Columbia Office of Research Services (www.ors.ubc.ca). We've structured our Web site so that, in general, you can visit the UBC CPD on the Web without identifying yourself or revealing any personal information. Once you choose to provide us personally identifiable information (any information by which you can be identified), you can be assured that it will only be used to support your relationship with UBC CPD. Please note that we do automatically collect internet tracking information.
1. What personally identifiable information are we collecting?
Information collected when visiting our web site:
We may collect the following information when you are visiting our Web site:

· The pages that were visited and the date and time of that visit
· The domain name (which typically identifies you Internet Service Provider) or IP address
We use this data in aggregate form to better understand how our site is navigated and to modify and improve the site.
We may also use “cookie” technology – a small amount of data sent to your browser and stored on your computer’s hard drive and/or on this site to track visitor's identification. We do use cookies when you register for online CPD. The cookies are used to remember that you have logged in to the web site. Since web page requests are stateless in that each request is a completely independent transaction something is needed to remember who you are between requests. No personal information is stored in the cookie. The session id is just a random string assigned each time you visit the portal and the user id is an internal id assigned to you when you registered for a course online. Our Website does not automatically gather any personal information from you.
If you do not wish to have cookies placed on your personal computer you can disable cookies though your internet browser. However, disabling cookies will prevent you from fully experiencing the services available on our website.
Information collected when you register:
If you register for one of our course offerings, we will collect personal information that you voluntarily share with us. If registering online this information is transferred using secure encryption technology. If a fee is charged and registrants can pay online, we ask for credit card information. These transactions are encrypted before transmission over the internet.
For all program registrations we collect personal information regarding contact information, address, email, phone number etc. This information is used for the purposes of facilitating the event you wish to register for and for communicating with you about future events we think you might be interested in.
Information collected when you participate in a CPD activity:
We ask participants to complete pre and post course surveys as well as evaluations of our CPD activities. This is to provide feedback to us on the educational value of the activity.

When you send us an e-mail or when you ask us to respond to you by e-mail, we learn your exact e-mail address and any information you have included in the e-mail.
2. What organization is doing the collecting
UBC CPD is the sole collector of any information provided by its website visitors and/or program participants. UBC CPD is a Division of the Faculty of Medicine, The University of British Columbia. More information on The University of British Columbia is available at www.ubc.ca.
3. How will we use the information?
UBC CPD uses your personal information specifically for program registration as well as to complete a transaction (payment). Unless you specifically request otherwise, each time you register for a program organized or delivered by UBC CPD we add your name to our database distribution list so that we may contact you through regular mailings, faxes, and/or email notifications about updates to our programs and services. This allows us to provide you with relevant and timely updates on CPD programs that align with your interests. We will only contact you if you indicate that you want to be notified.
4. With whom will we share the information?
The UBC CPD will not sell, rent, or lease your personally identifiable information to a third party without your consent. Unless we have your permission or are required by law, we will only share the personal data you provide online with other CME entities and/or business partners who are acting on our behalf to complete the CME activities listed on our website. Such CME entities and/or business partners, including those located in other countries, are governed by our privacy policies with respect to the use of this data.
UBC CPD will not use or share the personally identifiable information provided to us online in ways unrelated to the ones described above without letting you know and offering you a choice.
5. What choices do you have regarding collection, use, distribution of information?
Personally identifiable information can only be collected when you provide it. UBC CPD collects personally identifiable information about you during the registration process for:
(i) registration for CPD activities and other initiatives and/or
(ii) enrollment in the CPD distribution lists.
Currently the personally identifiable information collected is limited to the information required to facilitate the production of the program in question and/or to ensure that we are distributing information to you that is of interest to you.
You always have the option to refuse to provide the requested personally identifiable information but this will prevent you from being able to register for UBC CPD programs and from obtaining information about future UBC CPD programs and initiatives.
6. What kind of security procedures are in place regarding the information?
Protecting client privacy and personal information is a top priority. We make significant efforts to ensure the privacy of all personally identifiable information you provide to us. Access to all client information is restricted. Only employees whose jobs require accessing client information are granted database access or administrator level accounts on servers that contain such information. To prevent unauthorized access, maintain data accuracy, and ensure the appropriate use of information, we have put in place appropriate physical, electronic, and managerial procedures to safeguard and secure the information we collect online.
UBC CPD uses password protection, industry standard network encryption (128 bit SSL) when collecting or transferring sensitive data such as credit card information. We also use access logs and system monitoring to safeguard the confidentiality of client information. In addition, UBC CPD reviews its security procedures on an annual basis. Nonetheless, due to the inherent nature of the internet and related technology, we do not guarantee the protection of information under our control against loss, misuse or alteration. UBC CPD is committed to ensuring the security of your information.
Changes in Policy
As our internet site changes and grows, we may collect additional information or may use information in different ways. If our information practices change, we will modify this policy statement. When it does, we will post a notice on our home page to inform visitors of this change. If we substantially change our Privacy Policy as it relates to your personally identifiable information we will notify you using the email address we have on file, so it’s important to keep this information current.
